국내에 로봇수술이 도입된 지 10년이 지났다. 지난 2005년, 연세대학교 세브란스병원이 국내 최초로 로봇수술시스템 ‘다빈치’를 도입해 24례의 수술을 시행한 이래, 현재까지 2200례를 시행해 로봇수술은 놀라운 속도로 의료계에 그 영역을 넓혀왔다. 그 중에서도 난이도가 높다고 평가되는 전립선암의 경우 현재 로봇수술이 90%의 점유율을 차지할 정도로 비중이 커지고 있다.

로봇수술의 대가, 암 치료의 새로운 지평을 열다

국내 로봇수술의 첫 장을 열었던 연세대학교 의과대학 세브란스병원은 지난 2013년 11월, 세계 최초로 단일병원 로봇수술 1만례를 달성했다. 당시까지 국내 로봇수술환자가 총 2만 4207명이었다는 것에 비춰볼 때, 전체 환자의 약 42%가 로봇수술을 받기 위해 연세 세브란스 병원을 찾은 것이다. 또한 올 10월 기준으로 13,639례를 넘어서며 로봇수술 분야에서의 세계적인 명성을 이어오고 있다.

외과, 비뇨기과, 산부인과, 이비인후과 등 7개 임상과에서 각종 암수술 등에 활용하며 로봇수술 기술력과 노하우에 있어 독보적인 영역을 구축하고 있는 연세 세브란스병원에서도 최고의 실력을 바탕으로 가장 왕성한 활동을 보여주고 있는 이가 바로 비뇨기과의 최영득 교수이다. 그는 지금까지 연세 세브란스 병원 비뇨기과에서 수행한 4496례의 비뇨기과 로봇수술 중 2200례 이상의 수술을 혼자 도맡으며 명실상부한 ‘국내 최고의 로봇수술 대가’라 불리고 있다.

현재 전립선암의 수술은 다양한 방법으로 시행되고 있다. 개복수술의 경우 수십 년 전부터 시행되어 술기가 확립되어 있고, 장기간 축적된 수술 성적을 보유하고 있다는 장점을 가진다. 하지만 전립선이 골반 안쪽 깊숙이 자리 잡고 있고, 방광과 요도 연결 시 술자의 움직임에 제한이 따른다는 문제가 있어왔다. 이후 도입된 복강경수술은 개복수술에 비해 상대적으로 출혈이 적고, 수술 후 통증도 적어 빠른 회복을 기대할 수 있지만, 술기의 난이도가 비교적 높아 시행빈도가 높지 않은 상황이었다. 최 교수는 “로봇수술은 복강경 수술이 가지고 있는 장점과 더불어, 손 떨림의 보정, 3차원 카메라에 의한 깊이감 있는 넓은 시야, 로봇 팔을 이용한 자유로운 움직임으로 까다로운 수술도 안전하게 시행할 수 있다는 특징이 있습니다”라고 설명했다. 또한 로봇수술은 수술 후 결과적인 면에서도 개복에 비해 흉터가 작을 뿐 아니라 출혈과 통증이 많지 않고, 수술에 따른 스트레스가 적어 환자의 회복 속도가 빠르다. 최 교수는 “로봇수술은 최고의 정밀도와 안전성을 갖고 있습니다. 특히 암 제거 과정에서 소실될 수도 있는 신체기능을 보전할 수 있다는 점에서 앞으로 그 활용범위가 더욱 넓어지리라 예상합니다”라고 밝혔다.

세계적 수준의 로봇수술 노하우

오랜 경험과 노하우를 바탕으로 수많은 로봇수술을 성공적으로 이끌어오고 있는 최영득 교수는 로봇수술의 진가를 100% 활용하기 위해선 개복수술에 대한 깊은 이해와 해부학에 대한 해박한 지식이 바탕이 되어야 한다고 강조했다. 그는 “성공적인 로봇수술을 위해선 먼저 개복수술을 잘할 줄 알아야 합니다. 같은 수술이라도 개복수술에서의 풍부한 경험을 가지고 이를 로봇수술을 통해 조금 더 유리하게 활용하는 것과 단순히 로봇수술만을 연마한 뒤 시행하는 것은 큰 차이를 가집니다”라고 지적했다. 실제로 최 교수는 김대중 전 대통령을 비롯해 국내 정·관계 유명 인사들의 진료 및 수술을 집도한 바 있으며, 의료기기임상시험센터장, 비뇨기암클리닉 팀장, 로봇내시경수술센터장, 의료기기임상시험부장, 세브란스병원 비뇨기과장 등을 역임하며 쌓은 경험을 바탕으로 로봇수술을 이끌고 있다. 특히 전 세계적인 석학도 1시간 30분이 걸리는 로봇수술을 17분에서 30분가량의 짧은 시간에 신속하고 정확하게 시행하고 있으며, 이는 수술 시간이 길어질수록 환자의 몸에 걸리는 부담이 커진다는 점에서 환자의 빠른 회복에 큰 도움이 되고 있다. 하루에도 4~5개 정도의 로봇수술을 진행하고 있는 그는 암의 진척도나 환자 상태에 따라 로봇수술과 개복수술을 병행하며 환자를 위한 최선의 수술법을 선택하고 있기도 하다. 최 교수는 “최근에는 암의 초기진단이 가능해지면서 초기나 중기암의 환자들이 병원에 많이 방문하고 있습니다. 이런 분들은 간단한 절개만으로도 수술이 가능하고 회복이 빠른 로봇수술을 권장하고 있습니다. 하지만 암이 많이 진행되어 주변장기와 유착되어 있거나 절개부위가 확장된 말기암의 경우에는 개복수술이 더 좋은 효과를 발휘할 때도 있습니다”라고 설명했다.

최근 최 교수는 새로운 로봇수술 플랫폼인 ‘다빈치 Xi’의 표준 수술법 확립을 위한 연구에 힘쓰고 있다. 다빈치 Xi는 기존보다 가늘고 길어진 팔과 더욱 선명한 3D-HD 비전, 손쉬운 도킹 등의 장점을 가지고 있는 최신 기기로서 최근 세브란스병원은 국내에서 처음으로 ‘로봇 싱글사이트(단일공) 수술’을 시행해 주목받기도 했다. 최 교수는 “아직까지 로봇수술은 보험 적용이 되지 않아 고가의 비용이 든다는 단점이 있습니다. 그럼에도 불구하고 로봇수술 비중이 증가하고 있는 것은 그만큼 정확하고 안전하며 수술 후 부작용이 적다는 장점이 있기 때문입니다”라고 설명하며, 국내 로봇수술 분야의 발전과 확대를 위해서는 보다 체계적인 교육법의 확립과 우수인력의 양성이 동반되어야 한다고 역설했다.

병으로 고통 받는 환자들을 위한 더 나은 치료법 개발을 위한 연구에 최선의 노력을 다하고 있는 최영득 교수. 하루 새벽 4시부터 일과를 시작해 18시간 이상을 진료와 연구에 매진하고 있는 최 교수는 앞으로도 환자를 치료하는 의사의 본분을 다하겠다고 다짐했다. 해박한 지식과 풍부한 경험, 자신만의 노하우로 로봇수술 시대의 새 장을 열어가고 있는 그의 노력이 더욱 건강한 대한민국을 만드는 데 일조하게 되기를 기대한다.

An authority of robot medical operation opens a new prospect in cancer treatment

Severance Hospital opened the first chapter for robot operation in Korea and it had done the 10,000th operation in November 2013. This means that 42% of total 24,207 patients had operation at Severance Hospital. And the number of patients reached to 13,639 by October 2015 which made the hospital gain a global reputation. Urologist professor Youngdeuk Choi in particular is an authority in robot operation and he applied it in 7 medical areas including surgery, urology, obstetrics and ear-nose-and-throat clinic. He performed 2,200 operations out of total 4,496 in urology at the hospital. Currently, there are many operation methods for prostate cancer. Abdominal operation method for example has been practiced for many decades and is well settled now. But prostate is located deep in pelvis and limits the movement of operator when connecting bladder and urethra. Meanwhile, celioscope operation makes less bleeding and pain with faster recovery than abdominal operation but the level of operational difficulty is very high which made it to be a less popular method. Choi said "Robot operation improves the drawbacks of both methods while it holds shaky hand and provides wide view and free movement through the 3D camera and a robot arm. It is much more safer than previous two methods and causes less bleeding and pain. It also leaves a smaller scar and helps patients recover faster." Choi continued "Robot operation is the safest and the most precise method. It can remove the cancer cell without damaging the unaffected body part and for this reason it is expected to be more widely used.“

Providing world class robot operation knowhow

According to Choi, a doctor must have wide knowledge and deep understanding about abdominal operation and anatomy in order to use the advantage of robot operation by 100%. He said "If you have enough knowledge and experience in abdominal operation you can do robot operation much better." Choi performed robot operation for Former President Kim Dae-jung and numerous renowned politicians and businessmen. He also served as Director of the Medical Device Clinical Testing Center, Team Leader of Urological Cancer Clinic, Director of Robot Endoscopy Operation Center, Head of Medical Device Clinical Testing and Dean of Urology Department. He can carry out a robot operation within 17 to 30 minutes (normally takes 1 and a half hours) safely and precisely and this makes the patient recover faster. He does average 4 to 5 operations a day by using both robot and abdominal operations. Choi said "As we can find cancer in early stage, more and more patients in primary or mid stage cancer visit the hospital and robot operation is simple and ideal for them. If it is terminal cancer with the cell spread to the surrounded areas, abdominal operation might be more appropriate." Currently, Choi is working on establishing a standard operation method called Da Vinci Xi which is a new robot operation platform. It has a longer arm and clearer 3D-HD and easy docking than the existing robot. The first operation by using its 'robot single site (singleport access)' in Korea was done at Severance Hospital. Choi said "It's regretful that national medical insurance doesn't cover robot operation and the patient will have a huge bill as a result. But the reason it is getting popular nevertheless is because it is precise, safe and has less side effect." Choi added that we need a systematic training and foster talented doctors specialized in the field if we want to have an advanced robot operation technique in Korea. As an authority in robot medical operation, Choi wakes up at 4 a.m. and carries out his work and research for 18 hours everyday. Power Korea wishes Choi a great 2016.
