생의 마지막 순간 실천하는 숭고한 생명나눔 ‘인체조직기증’
“국내 인체조직 기증활성화 및 수급 확대를 통해 국민건강 향상에 기여할 것”

세계적인 인공고관절수술전문의로 손꼽히는 유명철 교수는 지난 2012년 아시아인공관절학회의 초대회장으로 추대된 바 있다. 환자를 치료하고 살리기에도 바쁜 그가 한국인체조직기증원(KFTD)의 이사장을 맡아 국내 인체조직기증 활성화를 위해서 불철주야 뛰고 있다. 한국인체조직기증원 본사를 방문해 그에게 직접 인체조직기증의 의미와 중요성에 대해 들어보았다. 
 
한국인체조직기증원이 보건복지부 지정 인체조직기증지원기관으로 선정된 소감이 어떤가?
지난 4월 6일 한국인체조직기증원(KFTD)이 드디어 공적 인체조직기증지원기관으로 발돋움 하게 되었다. 이제 한국장기기증원(KODA)이나 대한적십자사(혈액원)와 비슷한 지위를 가지게 되었으며, 매혈하던 혈액을 헌혈로 바꿨듯이 인체조직도 혈액이나 장기 등 타 인체유래물과 같은 공적 관리 체계를 구축할 수 있는 첫 발판을 마련하게 되어 감회가 남다르다. 

인체조직에 관심을 가진 배경에 대해 말한다면?
1976년 세계최초로 대퇴부 재접합 수술 성공 후, 약 1만 5천회에 걸쳐 파괴된 관절에 인공관절 수술을 했다. 관절을 못 쓰게 된 부분에 인공관절을 넣는 수술에서는, 뼈이식이 필요한 경우가 많으며, 특히 재수술시는 인공관절 주위로 뼈가 흡수되어 골결손을 보이는 현상이 많다. 환자의 뼈를 재활용하는 것으론 부족하여 결국에는 기증받은 뼈의 이식이 필요한 경우를 많이 경험했다. 그래서 2000년 초부터 뼈를 제대로, 안전하게 수급할 수 있는 인체조직기증의 필요성과 의미를 고민하기 시작했다. 인체조직은 기증자의 의지에 의해서만 공급받을 수 있는 생명의 일부분이므로, 기증의 숭고함과 취지를 알게 되면 많은 이들이 기증에 동참하게 될 것이다. 그래서 나를 비롯해 뜻있는 분들이 인체조직을 국내에 필요한 병원에 안정적으로 공급할 수 있는 공적기관의 필요성을 느끼고, 인체조직기증재단을 만들어 하나의 국가사업으로 진행하고자 했다.

국내 인체조직기증의 80% 이상을 담당하는 한국인체조직기증원의 역할과 업무가 궁금하다.
한국인체조직기증원은 지난 2010년 보건복지부의 인체조직전문구득 시범사업기관으로 선정되어 인체조직 기증 활성화, 국내 이식재 자급률 확대, 이식재 다양화 및 품질개선, 조직은행 전문 인력 양성 등 조직이식 분야 발전에 앞장서 왔다. 현재 서울성모병원 조직은행, 분당차병원 조직은행, 양산부산대병원 조직은행, 빛고을전남대병원 조직은행을 두고, 전국단위 구득사업을 진행하고 있으며, 이를 통해 이식재 품질관리 및 다양화, 안정적인 수급에 기여하고 있다. 또한 저소득층 환자들에게 이식재를 무상으로 지원하는 사회공언 사업도 진행하고 있다. 

이러한 운영 노하우를 기반으로 앞으로 국내 조직이식재의 안정적 수급을 위해 공적 인체조직기증지원기관으로서 △조직기증자 발굴을 위한 조직은행 및 의료기관 등 관련 기관과의 협력과 지원 △조직기증자 및 그 유족에 대한 지원 △조직기증에 관한 조사·연구 및 교육 △조직기증자 발굴을 위한 의료기관 종사자에 대한 정보 제공 및 교육 △조직기증지원기관이 보유한 조직 기증 관련 정보ㆍ통계 관리 등의 업무를 담당하게 된다.

현재 국내 인체조직 기증 현황은 어떤가?
인체조직기증이란 생존이나 뇌사 직후 이루어지는 장기기증, 헌혈과 달리 사후에 피부, 뼈, 연골, 인대, 건, 혈관, 심장판막 등을 기증하는 것이다. 국내 인체조직 수요량은 최근 3년간 연평균 7%씩 증가하는 반면 기증현황은 턱없이 모자란다. 미국의 경우 인구 백만 명 당 인체조직기증자 수가 100명, 스페인이 59명, 프랑스는 약 30명에 달하는 반면 한국의 경우 4.7명에 그친다. 세계보건기구(WHO)에서는 인체조직, 장기 등 인체유래물을 자국 내에서 자급자족하도록 권고하고 있는데, 열악한 국내 기증현황 때문에 국내 유통되는 인체조직이식재의 74%를 수입에 의존하고 있다. 인체조직이식재의 높은 수입 의존도는 수입 이식재의 안전성 논란과 환자의 고액 이식재비 부담이라는 부작용, 그리고 국가적으로 급박한 상황에 처했을 때 수급 안정성의 문제를 일으킬 수 있다. 

한국인체조직기증원의 역할이 점차 중요해지고 커질 것 같다. 앞으로 인체조직 분야 기증문화에 대해 관련 의료진으로서 나아가야 할 방향에 대해 말한다면?
인체조직 이식을 통한 조직재건은 국민 삶의 질과 직결될 만큼 중요하다. 대표적인 이식재인 피부와 뼈를 이식받는 환자들은 대부분 열악한 노동 환경, 성장 환경에 처한 저소득층 화상환자나 희귀암(골육종 등)으로 고통 받는 경우가 많기 때문이다. 게다가 우리나라에서는 장기와 인체조직 기증절차를 서로 다른 기관에서 진행해 기증한 고인들의 가족에게 부담을 주고, 운영 효율성도 저하된다. 윤리적인 문제와 인간에 대한 숭고한 존엄성도 충족해야만 한다. 이번 인체조직기증지원기관 지정을 필두로 안전한 이식재의 자급자족이 실현되고, 기증된 이식재가 혈액과 장기처럼 기증자의 생명나눔 정신에 맞는 가격체계를 갖출 수 있도록 기증 후 채취, 가공, 보관, 분배 단계를 관리할 수 있는 국가 차원의 관리기관과 공적 조직은행 마련이 시급하다고 본다. 스페인의 ONT(National Transplant Organization), 영국의 NHSBT(National Health Services Blood and Transplant), 캐나다 CBS(Canadian Blood Services) 등 현재 선진국에서는 인체유래물에 대해 철저한 비영리 원칙으로 국가 차원에서 운영한다. 

또한 의료현장에서 의료진들의 관심이 무엇보다 중요하다. 의료진 1명이 유가족에게 조직기증 권유 의향을 묻는 것만으로, 100명을 살리는 1명의 기증자가 더 늘어날 수 있는 것이다. 

인공관절 분야 세계적 권위자다. 지금의 자리에 오르기까지의 과정을 간단히 답해달라.
철도사고로 왼손을 잃고 의수를 끼고 평생 생활하신 아버지를 보며 자라나 정형외과 의사가 되었다. 경희대학교에서 41년간 교수로 재직하는 동안, 정형외과에서 관절과 관련된 많은 수술과 다양한 인공관절 관련 연구와 수술을 수행했다. 생활이 불편한 상지와 달리, 하지는 관절에 문제가 생기면 정상적인 활동이 불가능하다. 그들을 위해 1977년 인공관절연구소를 설립해 인공관절을 주도적으로 하게 되었고, 뼈 조직에 대한 필요성을 인식하면서 이 자리까지 왔다. 절단된 엄지손가락 자리에 발가락을 이식하고, 난도가 높은 표면치환술을 개발해 대퇴골괴사를 치료하기도 했다. 석좌교수로 있는 지금도 인공관절 수술이라면 경희의료원을 떠올리는 환자들이 많으시다는 점에서 보람을 느낀다. 또한 대학병원의 교수 재직 시절에는 28년간 ‘희망사회 만들기 의료봉사단’의 일원으로서 전국적으로 약 7만 5천여 명의 환자들을 진료하고 200여 건의 무료수술을 집도하는 무료봉사를 수행한 공로로 2001년 서울시민대상, 2004년 국제로터리클럽 ‘초아의 봉사상’, 2012년 보건복지부장관상(행복나눔인)을 비롯해 다수의 시상식에서 수상하는 영광스런 순간도 있었다. 

지난 2012년 한국, 중국, 대만, 일본, 홍콩 등 아시아 12개국 정형외과 인공관절 전문의사들이 모여 아시아 인공관절학회(Arthroplasty Society In Asia : ASIA)를 창립하고 초대회장에 선임되었는데.

초대회장으로 추대된 후 꾸준히 인공관절 연구, 개발, 교육 및 시술을 통해 국제 교류 활성화에 앞장서고 있다. 의료혜택을 받지 못하는 아시아 국가들에 도움을 주고, 젊은 의사들에게 교육시켜 의료기술을 전파하는 것이 목표다. 정형외과라는 전문 분야 속에서 의료인의 소임과 의료봉사활동이라는 배려와 나눔을 몸소 실천함으로써 인간 사회를 더욱 아름답고 행복하게 만들고 있다.

지난 30여 년간 이동검진차량으로 전국을 돌며 재능기부를 펼쳤다. 저서인 <휴먼닥터>를 통해 의사로서의 가치관 등을 서술했는데, 재능기부의 의미와 의사의 역할에 대해 말한다면?
나의 삶은 인공관절 전문 의사, 그리고 세상에서 할 일이 무엇인가를 찾는 인간의 역할 두 가지로 채워져 있다. 봉사가 청진기 진찰로 끝내지 않기 위해 엑스레이 및 검사 장비를 실을 이동검진차량을 마련해 전국의 환자들을 발굴하고 무료수술 등을 했다. 강원도 고성에서 제주도 서귀포까지 전국을 순회하면서 약 7만 5천여 명을 진료했고 대학에 있으면서 평생토록 연구와 교육에 심혈을 기울였다. 의사이다 보니 아픈데도 경제적 상황이나 신체적 여건으로 병원에 올 수 없는 사람이 많음이 안타까웠다. 건강과 질병은 사람의 삶을 지탱해주는 목적과 의미를 결정하는 요소다. 그래서 현장에 나가 그런 사람들의 고충을 나누고 의료기술을 통해 도움을 줄 수 있다는 것은 의사로서, 그리고 인간으로서 반드시 해야 할 일이라고 생각한다. 그래서 한국을 넘어 중국에도 매년 무료진료를 나가고 있으며, 사할린, 베트남, 인도네시아 등 아시아 곳곳을 돌면서 시간이 허락하는 대로 진료하고 있다. 

인체조직 기증에 대해서도, 사후에 화장을 하면 재가 된다. 하지만 내가 이 세상에서 생명을 다 하고도 생명이 꺼지지 않고 간접적으로 연속될 수 있다면 그 자체로 인간사회를 아름답게 만드는 촛불이 되는 것이다. 이런 일을 이미 알고, 실천하는 사람도 있지만 아직 모르는 사람도 많다. 우리가 이것을 일깨워 그 사람이 고귀한 일에 동참할 수 있도록 만드는 일은 굉장히 중요하다. 사람과 삶, 세상에 태어난 의미, 인생을 마감하는 죽음을 더욱 이성적으로 생각하고 사회 전체가 그런 운동에 동참한다면 더욱 가치 있지 않을까. 어떻게 보면 생명의 씨앗을 잇는 것이고, 나의 정신과 뜻은 다음 세대에 의미 있게 전달되는 것이다. 후배 의사들에게도 이 점을 늘 강조하고 있으며, 의사라는 직업이 사회생활을 하는 수단보다는 사회로부터 받은 임무, 인간으로서 해야 하는 책무임을 이해하고 사회에서 질병으로 고통 받는 많은 사람들에게 베풀고 나누는 역할을 하길 바란다. 아름다운 생각을 베풂으로 나눈다는 꿈과 목표를 가지고 의료의 길을 걷길 바란다. 그런 생각을 가지고 의료활동을 하면 사회로부터의 존경이 따라올 뿐 아니라, 수술을 잘하는 고도의 기술자를 넘어 환자를 위한 진정한 명의로 거듭날 수 있는 것이다. 

마지막으로 독자들에게 하고 싶은 말은?
아직도 주변에는 적합한 인체조직을 기증받지 못해 고통 받고 불구가 되는 수많은 환자들이 있다. 평균수명 증가와 교통사고, 산업재해 등 조직이식이 필요한 사례가 증가하면서 이러한 환자들은 더욱 늘어날 것이다. 소명의식을 가진 의료진들의 적극적인 동참과 지속적인 법제도의 개선, 그리고 온 국민적 관심과 참여가 하나로 묶어질 때 선진국과 같은 인체조직기증문화 확립과 생명 나눔의 선순환을 이루어 낼 수 있을 것이다.

How do you feel about Korea Foundation for Human Tissue Donation being appointed as a human tissue donation support organization by the Ministry of Health & Welfare?
I'm happy that KFTD was appointed as an official human tissue donation support organization by the Ministry of Health & Welfare on 6 April. It means that we are now equal to Korea Organ Donation Agency and Korea Red Cross (Blood Bank) and we can be able to build a human tissue management system like them.  

What prompted you to develop a fascination with human tissue?
I carried out 15,000 artificial join surgeries for damaged joints after I succeeded a femoral reconjugation surgery for the first time in the world in 1976. Artificial joint surgery accompanies bone graft in most cases and reoperation can cause a bony defect due to the bone being absorbed into the artificial joints area. I strongly felt a need for bone donation as the reuse of patient's bone is limited. I started working on it from the year 2000 and gathered those who felt the same need as I felt to form an organization to supply human tissue for domestic hospitals. Human tissue donation is a really noble thing to do and it can only be done with donor's good will. 

You take more than 80% of supplying domestic human tissue. What is your role?
We have been exerting our effort in activation of human tissue donation, expanding the self-sufficiency of transplant materials and its variety & quality improvement as well as fostering specialists relating to human tissue. We established Tissue Bank in Seoul St. Marry's Hospital, Budang Cha Hospital, Busan National University Yangsan Hospital and Chonnam National Unveristy Bigoeul Hospital and are doing subscription project nationwide to promote management, diversity and stable supply of tissue materials. We also carry out free tissue materials project for patients from low income families. Our role in a nutshell is to do △support and cooperation with tissue banks and medical institutions to find donors △R&D and training on human tissue donation △providing information & training for health care workers to promote the number of donors △management of human tissue donation organizations & the information.

What is the present condition of domestic human tissue donation?
Human tissue donation means that donors donate their tissues such as skin, bones, cartilages, tendon, sinews, blood vessels and heart valves after they died. The demand of domestic human tissue has increased by 7% yearly for the last 3 years but the number of donations was far too little. The donors in the US, for example, are 100 people per 1 million, 59 in Spain and 30 in France but Korea stays at 4.7. WTO recommends each country to be self-sufficient in organ & human tissue donation but we have to import 74% of the tissue from abroad. The problem is that this can raise safety issue, implant cost and stability of supply in Korea.  

It seems like your role is becoming very important. What do you think is your opinion about public recognition of human tissue donation in Korea?
The donation is closely related with the public health and their quality of life. Most human tissue beneficiaries are from low income families or patients from rare diseases. In addition, we proceed organ and human tissue donation procedure from each different organization unlike the advanced countries which cause administrative ineffectiveness as well as a burden to the families of the donors. Ethical issue and dignity also need to be respected. Therefore, I think it is urgent to establish a national level management organization and tissue bank to effectively control the necessary procedures from donation, collection, processing, storage and distribution of the materials at a reasonable cost. Good examples include Spain's ONT(National Transplant Organization), UK's NHSBT(National Health Services Blood and Transplant) and Canada's CBS(Canadian Blood Services) which are managed by a non-profit public organization. The role of staff in a hospital also is very important as they can save 100 lives by encouraging one of donor's family members to participate in donation. 

You are a world-wide authority on the field. How did you get here?
I grew up by watching my father having a prosthetic arm which he lost by a train accident and I decided to be an orthopedic surgeon. I have done a significant amount of joint related operations during my 41 years of service in Kyunghee University as a professor. People get limited of doing normal activities if they have joints problems in the lower half body. I established an artificial joint research institute in 1977 and developed my interest in bone tissue. I implanted a toe in the place of lost finger and developed a surface replacement surgery to cure the thighbone necrosis. I feel worth when people think of me when they heard the words 'artificial joint surgery'. As a member of 'Hope Medical Team', I treated 75,000 patients and did 200 operations all free of charge for 28 years. For my contribution, I received numerous awards including '2001 Seoul People Award', '2004 Choa's Service Award of Rotary International and '2012 Minister Prize of Health & Welfare. 

Artificial joint medical specialists from 12 Asian countries formed Arthroplasty Society In Asia (ASIA) in 2012 and you were appointed as the first president of the society.
It is an honor to be the first president of the society. The society is taking the initiative in activating international exchange through related R&D, training and operations. The goal of the society is to provide help for the developing countries and to train young doctors to spread medical techniques to the world. It is important to do your duty as an orthopedic surgeon but providing social services is equally so to make a better world. 

You have carried out a talent donation for the last 30 years on an itinerant medical check-up car and wrote a book called 'Human Doctor'. What is the role of a doctor in terms of social contribution? 
I have two roles in my life. One is as an orthopedic surgeon and another as one of society members. I used a car loaded with medical equipments and traveled the whole nation to give free medical services for 75,000 people who are in need in the hope of making a better society. I also do this free itinerant medical check-up in China, Sakhalin, Vietnam and Indonesia whenever I have a spare time. Human tissue donation in particular is very meaningful. You can share the sublime moments by saving other's lives with your tissue. It connects one life to another and one generation to next generation. I emphasize more on the duty we have to perform as a member of society rather than the mechanical role as a doctor. The difference between a great doctor and doctor as a job comes from this. 

Lastly, what would you like to say to Power Korea readers?
There are many patients who are desperately waiting for your noble tissue donation. It is notable that the number of those who are in need of the donation only can go up considering the fact that we are entering the aging society not to mention the unexpected accidents. Therefore, we need your participated right now so that we can together build a better society and the world.
