지난 2000년 경기 일산에서 설립되어 국내 최초로 암 관련 연구, 진료, 교육 및 국가 암 정책 지원 기능을 갖춘 국립암센터는 최근 보건복지부 공공보건의료계획 시행결과평가에서 3년 연속 최우수기관으로 선정되었다. 국립암센터는 암에 관한 연구를 수행하는 연구소, 암을 진단하고 치료하는 부속병원, 보건복지부의 암 관리 사업정책을 지원하는 국가암관리사업본부, 그리고 2014년 3월 개교한 국제암대학원대학교로 구성되어 있고 국립암센터의 연구소는 한국인의 주요 암의 발생률을 낮추고 암 치료율을 제고할 수 있는 이행성(Bench to Bed) 연구뿐 아니라 민간에서 수익성 등을 이유로 수행하기 어려운 희귀 난치성암, 소아암 등의 공익적 연구에도 힘쓰고 있다. 기술이전 성과도 뛰어나 올해 3월 면역치료기술을 33억 원에 이전하는 등 올해에만 총 63억 규모의 기술이전 실적을 달성했다. 월간 파워코리아는 개원 15년이라는 짧은 시간 동안 놀라운 성과를 보여 준 국립암센터에 2014년 6대 원장으로 취임하여, 연구와 진료, 경영 모든 분야에서 국립암센터의 중장기적 발전을 수립해 순조롭게 이끌고 있는 이강현 원장을 만나 전문 암센터의 존재가치와 한국 암치료기술의 현황을 짚어 보았다. 

변화하는 암 치료·연구 패러다임에 맞춘 개개인 맞춤치료로 국민건강 수호
이 원장은 최근 국내 정밀의료 실현을 앞당기기 위해 왕성한 추진력을 바탕으로 외연 확대와 내실을 다지고 있다. 암은 유전자의 이상으로 생기는 질환이므로 임상정보와 함께 유전자 정보를 바탕으로 진단·치료하는 정밀의료가 가장 빛을 발할 수 있는 대표적인 분야이다. 정밀의료는 개인 맞춤식으로 치료하는 전략이다. 정밀의료가 실현되면 암 환자의 의료, 임상 정보뿐 아니라 유전체 분석을 통한 유전학적 진단과 가족력, 생활습관 정보, 환경적 특성 등을 종합적으로 분석해 가장 효과적인 치료 방법을 제시하게 된다. 뿐만 아니라 장기적으로는 치료 중심에서 예방·관리 중심으로 의료 패러다임을 바꾸게 될 것이다.   

이 원장은 작년부터 암 임상 데이터를 수집하고 빅데이터를 구축할 국가암데이터센터추진단, 대내외 정밀의료 협력사업을 추진할 암정밀의료추진단 및 정밀의료를 중점적으로 연구하는 정밀의학연구부를 설치해 의료 패러다임의 변화를 준비해 왔다. 또한 차세대유전체분석실을 신설하고 임상유전체분석과 암종양은행의 기능을 강화하는 연구소 조직개편을 통해 내부조직 정비를 마치고 암 빅데이터 구축을 시작했다. 또한 국가전략 프로젝트를 통해 국립암센터가 정밀의료 기반 암 진단 방법 및 치료방법 개발을 선도할 수 있도록 보건복지부 및 관련 기관들과 긴밀하게 협력하여 노력하고 있다. 국립암센터는 여러 기관과 함께 재발 또는 전이된 진행성 폐암, 위암, 대장암 환자 등 1만 명의 유전체 정보를 분석하고 표적 유전자가 확인되는 환자를 대상으로 신약으로 표적치료를 시행하는 다기관 정밀의료 기반의 임상시험 치료를 진행할 예정이다.   

다학제적 통합진료 도입하여 전문성 높이고 최고의 치료과정 도출
부속병원은 국내 다학제적 통합진료의 효시로서 11개의 질환별 진료센터와 5개의 기능별 진료센터를 운영하고 있다. 이 중 양성자치료센터는 국내 최초로 양성자치료기를 도입한 것으로 유명하다. 양성자치료는 주변 정상조직의 손상을 최소화하면서 암세포만 정확하게 타격하는 최첨단 방사선치료기이다. 이 원장의 중점 추진 사업 중 하나가 부속병원의 증축이다. 지금의 공간으로는 증가하는 환자와 공공의료기관으로서의 역할을 충분히 담아내기에 부족한 측면이 있다. 현재 기획재정부 예비타당성 조사를 거치고 있는 증축 계획이 완성되면 다른 병원들이 수익성을 이유로 투자하고 있지 않은 호스피스와 소아암 병동이 주로 확대 증설된다. 또한 보다 섬세한 케어가 필요한 여성암 환자를 위한 전문 병동을 설치해 여성암 환자의 효율적 진단과 치료, 조기 사회복귀를 돕는 등 보다 여성 친화적이고 특화된 서비스를 제공할 예정이다. 

국가 암 관리계획을 통해 암을 장기적· 종합적으로 관리 
국립암센터는 국가암관리사업본부를 통해 국가 암 관리계획 등 암 정책을 개발하고 암 관리 사업을 활발하게 수행하고 있다. 또한 중앙암등록본부를 통해 세계 최고 수준의 암 발생 통계를 산출하고 있다. 작년 중앙암등록본부는 최근 5년간(2009-2013년) 발생한 암환자의 5년 상대생존율이 69.4%에 이른다는 통계를 발표해 국민들에게 조기진단과 적절한 치료를 통해 암도 관리할 수 있다는 희망의 메시지를 전달했다. 또한 보건복지부와 근거 중심의 암 관리 정책을 구현하기 위해 제3차 국가암관리 종합계획을 마련해 추진 중에 있다. 

전문대학원 승격, 암 정복 리더 배출하는 터전 될 것
국제암대학원대학교는 국립암센터가 그 동안 쌓아온 암 관리와 연구의 노하우가 집약된 교육기관이다. 국내뿐 아니라 최근 암 부담이 폭발적으로 증가하고 있는 아시아·아프리카 개발도상국 출신의 암 전문 인력을 양성하기 위해 2014년 3월 개교했다. 이 원장은 석사과정만 있던 국제암대학원대학교가 세계적인 교육기관으로 자리매김할 수 있도록 올해 9월 교육부로터 새롭게 인가를 받고 박사학위 과정을 설치할 수 있는 전문대학원으로 전환했다. 이는 국제사회와 정부가 암 전문 인력을 꾸준히 필요로 하기 때문만 아니라 개교 이후 2년간 국내외 우수학생을 선발해 안정적으로 대학원을 운영한 전문성을 인정받아 가능했다. 이 원장은 국제암대학원대학교의 역사가 깊어질수록 암 분야의 국제무대에서 활약하는 동문들을 쉽게 만날 수 있을 것으로 기대했다. 이 밖에도 국립암센터는 암 분야의 국제적 협력 네트워크를 구축하는데 앞장서고 있다. 미국 국립암연구소, 일본 국립암센터 등의 세계적인 암 전문기관과 MOU 체결 이후 협력 관계를 유지하고 있는데, 최근 정밀의료, 암 단백유전체학(cancer proteogenomics) 등의 분야에서 긴밀하게 협력하기로 약속하며 국내 정밀의료 실현을 앞당기기 위해 박차를 가하고 있다. 

암센터라는 말조차 낯설었던 2000년에 최초의 암센터로 설립되어 국민들의 암 부담을 줄이기 위해 15년 넘게 달려온 국립암센터. 공공기관으로서의 공공성을 추구하면서도 스스로를 뛰어넘는 자기 혁신을 통해 최고를 향해 나아가는 국립암센터의 중심에는 이강현 원장을 비롯한 국립암센터 직원들의 노력이 있었다. 이 원장은 소명의식, 자긍심, 변화와 혁신, 소통, 솔선수범이라는 핵심가치를 몸소 실천하기 위해 노력하고 직원들과 허심탄회한 소통을 위해 항상 원장실 문을 열어놓는다는 말로 인터뷰를 마무리했다.     

이렇듯 국립암센터를 성공적으로 이끌며 2016년 한국을 빛낸 최고인물 대상을 수상하기도 한 이 원장은 앞으로도 더욱 내실을 기하며 세계최고의 암센터를 향한 행보를 멈추지 않을 것이라고 강조했다.

Customized cancer research and treatment guard people’s health
Lee Kang-hyun, President of the National Cancer Center, recently is exerting his effort in putting forward Korea’s precision medicine through expansion of its scope and ensuring internal stability. Cancer is a disease occurring by gene abnormality and it is the area where precision medicine shines the brightest. Precision medicine diagnoses and treats diseases based on clinical and genetic information and it is a treatment strategy customized for individuals. Realization of precision medicine can bring up the most effective treatment methods through comprehensive analysis on treatment information of cancer patients, clinical information, genom analysis-driven genetic diagnosis and patient’s family history, habit and environment. It might also turn the existing treatment-centered paradigm to the preventive and management-centered in a long term perspective. For this, Lee has introduced the National Cancer Data Center for building clinical big data for cancer, the Cancer Precision Medicine Bureau for cooperative precision medicine projects and the Precision Medicine Research Lab for intensive studies on precision medicine. Lee also introduced the Next Generation Genom Analysis Lab and reorganized the research centers of the NCC to strengthen the functions of clinical genom analysis and Cancerous Tumor Bank in preparation of building cancer big data. In order for the NCC to lead development of precision medicine-based cancer diagnosis and treatment methods through national projects, Lee is working closely in cooperation with related organizations including the Ministry of Health and Welfare. The NCC will be analyzing genom information of 10,000 patients with reoccurred or metastasized cancer, lung cancer, stomach cancer and cololectal cancer in cooperation with various organizations and doing multi-organizational precision medicine-based clinical test treatment that can give targeted treatment with new medicines for the patients with targeted gene among those 10,000.

Multidisciplinary integrated care system raises professionalism
and the best treatment results
The NCC’s multidisciplinary integrated care system started at its affiliated hospital. The hospital is running 11 treatment centers customized to each disease and 5 functional treatment centers. The Proton Treatment Center especially is famous for introducing the Korea’s first proton treatment device which is a cutting edge radiation treatment device that could precisely strikes the cancer cell only while minimizing the damages to the normal tissues around. One of the core projects of Lee is extension of the hospital as the existing capacity is not good enough to accommodate an increasing number of patients. The extension plans currently is being screened for preliminary feasibility by the Ministry of Strategy and Finance. A hospice and a paediatric cancer ward, which many other hospitals tend to avoid for low profitability, will be added when approved. A specialized ward for women cancer patients will also be added for sensitive and specialized diagnosis and treatment and it will promote an early rehabilitation to the society. 

Integrated long-term national cancer management for cancer patients
The NCC brings up national cancer policies and management plans and actively carries out cancer management projects through the Division of Cancer Management Policy. The Division of Cancer Registration & Surveillance, on the other hand, compiles global level cancer statistics. A recent statistics of the division reports that the 5 year relative survival rate among the cancer patients between the years 2009 and 2013 is 69.4%, meaning that cancer patients now can manage the disease with earl diagnosis and appropriate treatments. Currently, the NCC is pushing forward the 3rd Comprehensive Plan of National Cancer Management in order to realize the evidence-based cancer management policies of the Ministry of Health and Welfare. 

National Cancer Control Institute fosters talented cancer specialists
The National Cancer Control Institute is where the NCC’s cancer management and research knowhow are concentrated. The institute opened its door in March 2014 in order to foster talented cancer specialists from developing countries in Asia and Africa where the burden of cancer treatments are explosively increasing. The institute used to run master’s courses only but elevated its status to a specialized graduate school in September this year by obtaining an authorization for running the doctor’s courses from the Ministry of Education. The ministry recognized the institute’s capability in fostering talented students home and aboard for the last 2 years through a stable operation. Lee predicts that he will observe more alumni of the institute who are taking an active part in the international stage as the history of the institute deepens. Meanwhile, the NCC is taking initiative in building international cooperation networks for cancer researches and treatments. The NCC signed a MoU with the National Cancer Institute US and the National Cancer Center Japan and they are closely working together in the fields of precision medicine and cancer proteogenomics. Since its establishment in 2000, the NCC has only pursued benefit of public health and innovation in management for the last 15 years. And behind of this remarkable growth lies the cancer specialists of the NCC’s passion and effort as well as their motto ‘from the first to the best’. Communications for Lee is the key to elevating sense of duty, pride, change and innovation of the cancer specialists of the NCC through which they can set up an example in the field. The doors for communications are always open, Lee says. 

In recognition of his achievements, Lee recently has received the Great Prize for Proud Koreans Who Brought Honor to 2016 Korea.
