환경오염 및 잘못된 음식과 많은 스트레스 그리고 나쁜 생활습관 등으로 우리나라는 여전히 OECD국가 중 암 발생률이 최상위권이다. 기존의 현대의학에 의한 수술, 항암요법 및 방사선 치료로 현저하게 생존률이 높아진 것은 사실이지만 많은 후유증과 부작용이 따르게 된다. 이러한 때에 환자의 면역력을 강화하고 체력을 보강하여 보다 높은 삶의 질을 누리게 하는 통합 암치료가 절대적으로 필요한 시점이다. 기존 현대의학과 통합의학을 상호보완하여 암치료의 어려운 점을 극복하고자 하는 대한통합암학회 최낙원 이사장으로부터 자세한 설명을 들어보았다.

현대의학과 전통의학을 포함한 다양한 통합암의학의 장점으로 암 환자들에 도움

고령화시대의 건강한 삶을 위협하는 암, 대개는 수술과 항암요법·방사선치료 등으로 치료하는 것이 일반적이지만, 치료부작용 및 후유증으로 기력저하, 악액질, 통증, 면역력 저하등에 의한 각종 감염 및 패혈증 등으로 어려움을 겪는게 현실이다. 치료효과가 있다고 인정된 모든 치료와 의술들을 과학적으로 근거를 연구하고 세계적으로 인정받은 시술들을(심신요법, 운동요법, 식이요법, 침구요법, 통증치료, 온열치료 및 파이토케미칼로 알려진 천연항암물질요법, 니시요법, 전통의학)을 이용하여 치료하는 것이다. 다양한 학제간의 치료들을 서로 이상적으로 조합하고 적용하여 현대의학치료와 상호보완 및 강화를 도모함으로써 이상적인 치료를 유도하고 따라서, 건강하고 높은 삶의 질을 누리게 하는 것이며 이는 그동안은 수동적인 요양치료로부터 적극적인 보조치료로의 전환을 의미하는 것이다.

현대의학과 한의학 의사자격을 모두 보유한 최낙원 대한통합암학회 이사장은 이러한 통합의학을 이용한 암치료연구를 위한 학회를 출범시켰다. 그는 “현대의학의 치료를 더 강화하고 보완하는 의학으로서 근거중심의 과학적 통합의학 암치료가 필요하며 이런 치료들은 세계적인 학문적 근거를 꼭 가져야만 인정받는 치료반열에 들어갈 수 있으며, 수술과 항암요법을 거친 환자를 회복시키고, 화학요법과 방사선요법의 후유증 등을 적극적으로 해결하기 위한 치료법”이라고 한다. 통합의학은 항암치료에서 환자의 회복력을 향상하는 면역암치료와 줄기세포 치료등을 미래 항암치료의 중심으로 삼고 있고, 이는 그간 치료들에 비해 재발과 부작용이 적은 장점이 있으며 미래 암치료에 있어서 근간이 되기 때문이다. 종양의 발생기전을 면역체계 손상으로 항원-항체 반응을 통한 여러 대식세포 및 수상돌기세포 등을 이용하여 T-cell을 포함한 다양한 면역력을 강화시키는 물질들을 이용해 종양세포만 공격하게 하는 것이다.

이러한 치료에는 항체, 인터페론, 백신, T-reg cell, TLR 및 Interleukin 등을 포함하는 것이다. 물론 vitaminA, vitaminD, α-lipoic acid 등을 보조적으로 추가하여 암세포 증식과 전이를 막게하는 것이다. 이러한 면역체계를 이용한 치료들이 미래암의학으로 발전하게 될 것이다.

암환자에게 항암제 치료와 방사선 조사시 온열요법을 시행하면 면역력을 올려 종양의 크기를 줄이고 사멸시키며 저체온증세를 완화해 치료효과를 높이는 것이다. FDA 및 NIH 등에서 인정한 항암 식물약성물질 및 영양물질 등을 임상에 적용하고 NCCAM 등에서 정한 암치료 가이드라인으로 적용하여 치료하는 계기가 될 것이다.

“현대의학과 통합의학이 결합한 암 치료에 대한 인지도가 낮은 것은 사실이다. 이러한 다양한 암 치료들을 이미 적용하고 있는 미국, 독일보다는 한국이 늦게 시작한 것이다. 최근 중국의 천연물 약리학자인 투유유(屠呦呦,85) 교수의 '칭하오쑤'(靑蒿素·아르테미시닌)를 임상 및 과학적으로 증명하여 노벨의학상까지 수상하게 된 것은 바로 파이토케미칼을 세계적으로 인정받은 단계로 이끈 좋은 예라 할 수 있다.

최근 다양한 암 환자의 요양 및 재활을 맡고 있는 요양병원협회와 공동 주최로 이번 학술대회를 진행하며 앞으로 학회지 정기 발간, 연수교육 및 학술대회개최 등으로 암 치료에 관련된 의료인등을 상대로 교육을 실시할 것으로 계획하고 있다. 지난 11월 29일 가톨릭의대 서울성모병원에서 개최된 국제학술대회에서는 북경 광안문병원의 폐암 전문인 중국의 린홍셩 교수, 파이토케미칼 전문인 리지에 교수, 동,서 결합형 의학으로 암 치료하는 션이에화 교수, 통합의학으로 암 치료하는 이토 토시노리 교수, 온열요법의 치료기전 및 임상효과에 대한 독일의 얀 로센리흐 박사, 후세인 사인바스 박사, 러시아 암 센터의 세르게이 고르데레브 박사가 참여하여 국제 통합암학술대회를 성대히 치뤘다. 이미 미국의 유명 암센터인 MD Anderson 및 Sloan Kettering 등에서도 통합의학을 도입해 환자 몸에 독성이 적고 면역세포를 보호해 완치율을 높이는 치료를 하고 있다. 현대의학과 기능의학, 그리고 전통의학을 포함한 통합의학으로 서로의 장점을 합쳐 새로운 개념의 암 치료의 패러다임과 가이드라인이 도입되길 바라며, 그 일에 대한통합암학회가 앞장서 이끌어가길 바란다.

Introducing guidelines for anti-cancer treatment with scientific integrative oncology
Choi suggests a new paradigm for total cancer treatment
Treating cancer patients with conventional medicine and integrative oncology

Surgery, chemotherapy and radiotherapy are commonly used to treat cancer. But they accompany many side effects and complications, such as loss of strength, cachexia, pain and deterioration of immunity including infection and sepsis. To improve and cure the disease, doctors are using globally approved procedures including mind-body therapy, exercise, diet therapy, acupuncture, pain modulation, hyperthemia, natural anti-cancer substance (phytochemical) therapy, Nishi medicine and traditional medicine. And many specialists also have combined and applied these treatment methods to cure the disease. The recent trend shows that it is shifting to active supportive treatment from passive palliative care. Rak won Choi is a qualified doctor in both fields of Conventional and Traditional Korean medicine. He established the Korea Society of Integrative Oncology in order to carry out researches on anti-cancer treatment using integrative medicine and is currently serving as the President of KSIO. He says “If we are to improve and strengthen the existing cancer treatment, we need both academically and scientifically proved integrative oncology. It can bring back the patients who already have gone through surgery and anti-cancer treatment to recover and can actively improve the side effects of chemotherapy and radiotherapy.” The integrative oncology takes ‘immune anti-cancer treatment’ and ‘stem cell treatment applications’ as two core elements to reduce recurrence and side effects more than the existing treatments and it will be the foundation for future cancer treatment. It uses immune-mechanism based T-cell and various other substances that can strengthen immune-function in order to attack the tumor cell with macrophage and dendritic cells. The treatment includes antibody, interferon, vaccine, T-reg cell, TLR and Interleukins. Also, vitamin A, vitamin D and α-lipoic acids are added to modulate T-cell systems and eventually prevent proliferation of cancer and its metastasis. These enhancement and modulation methods using immue-system will be expected to have main roles in future integrative oncology. Meanwhile, anti-cancer phytochemicals and hyperthermia can increase immune-functions and reducing the size of the tumor as well as relieving the hypothermia. Anticancer herbal drugs and nutrients (approved by FDA and NIH.) are applied to patients. And guidelines standardized by NCCAM for cancer treatment will be adopted in clinical use. Choi says “The recognition on combined treatment of modern medicine and integrated medicine is still low in Korea when compared to the US and Germany where the method has already been in use. The Chinese Nobel Prize winner Tu Youyou (natural pharmacologist)’s proving artemisinin through scientific and clinical test is a really good example for phytochemical to be recognized globally. Meanwhile, KSIO and the Korean Association of Geriatric Hospitals (rehabilitation of cancer patients) held the International Integrative Oncology Symposium and are planning to publish medical journal and provide training courses for doctors. The symposium on 29 November at the Catholic Univ. of Korea Seoul St. Mary’s Hospital were attended by lung cancer specialized Prof. Lin Hongsheng of Beijing Gwanganmun Hospital (China), phytochemical specialized Prof. Li Jie, combined treatment specialized Prof. Shen Yehua, integrative medicine specialized Prof. Ito Tosinori, thermotherapy specialized Dr. Jan Rosenlicht and Hϋseyin Sahinbas (Germany) and Dr. Sergei S. Gordeyev of Russian Cancer Center. Integrative oncology is already used in the famous cancer centers of the US such as MD Anderson and Sloan Kettering in order to raise full recovery rate. Power Korea hopes that Choi and KSIO will lead a new paradigm and guideline of cancer treatment through integrative oncology in the future.
