세계 속의 명품의학 ‘한의학’의 기틀을 다지다

최근 의약계에서 가장 뜨거운 논란이 되고 있는 것은 바로 ‘천연물신약’이다. 지난 8월 20일 서울행정법원 2심에서 지난해 ‘천연물신약 고시’를 무효로 판결했던 1심을 뒤집고 “문제가 없다”는 판결을 내린 것이다. 이미 지난 7월 감사원의 결과보고를 통해 정책 전반에 대한 문제점이 노출되며 비판의 대상이 됐던 천연물신약 정책이 다시금 도마 위에 오르게 된 것이다.

천연물신약 정책, 전면적인 재정비 필요
대한한의사협회는 그 간 1조 4,000억 이상의 재정이 낭비되고, 국민 건강을 심각히 위협하고 있는 천연물신약 정책에 대한 재정비를 촉구하는 데에 앞장서 왔다. 특히 현재의 천연물신약 정책이 본래의 취지와 달리 왜곡되고 있다는 점에 심각한 우려를 표하며, 전면적인 개정을 통해 보다 발전적인 방향으로 나아가야함을 강조해왔다. 대한한의사협회의 박완수 수석부회장은 “당초 천연물신약 정책은 우리나라의 자생식물을 이용해 아스피린이나 탁솔과 같은 특정 성분을 추출, 세계시장에 통할 수 있는 블록버스터급 신약을 개발한다는 취지로 출발했습니다. 그러나 수천억 원이 투자됐음에도 성과가 미흡하자 식품의약품안전청이 수차례 고시변경을 진행해왔고, 이후 오히려 국민 건강을 위협할 수도 있는 엉터리 천연물신약 정책으로 변질되고 있습니다”라고 설명했다.

이에 따라 협회는 대정부활동의 일선에 서서 그릇된 정책을 바로잡기 위한 활동을 벌여왔다. 특히 기존 정책 재정비를 통한 제대로 된 천연물신약 개발의 토대를 마련함과 동시에 세계전통의약시장을 선도하기 위한 한약제제 사업을 함께 발전시키는 투 트랙 전략이 필요하다는 제안을 내놓았다. 박 부회장은 “천연물신약 시장은 세계적으로도 활발한 연구가 진행되고 있는 차세대 유망산업 가운데 하나입니다. 그러나 많은 이들이 간과하고 있는 것 중 하나는 전통의약시장 역시 세계인들의 높은 관심 속에 발전을 거듭하고 있다는 사실입니다”라고 설명하며, “가까운 중국의 경우 전통의약인 ‘중의약’을 통해 매년 수출로만 4조원이 넘는 외화를 벌어들이고 있지만, 우리 한의학은 국내시장에만 머물러 있는 상태입니다”라고 밝혔다. 

실제로 중국은 정부차원에서의 ‘중의약 발전’을 기치로 내걸고, 연구개발과 수출판로 개척을 위한 지원에 적극적인 자세를 취하고 있다. 반면 우리나라 한약제제산업의 경우 각종 불합리한 규제와 법·제도의 미비함으로 인해 2002년 3,654억 원을 기점으로 2005년 2,192억 원, 2009년 1,628억 원 등 지속적인 하락세를 기록하고 있는 상태다. 더욱이 국민건강보험에서 보장해주고 있는 약제비용(양약)은 연간 10조가 넘지만, 한약제제의 보험급여는 연간 300억원에도 미치지 못하고 있다. 박 부회장은 “천연물신약은 천연물신약답게, 한약제제는 한약제제답게 개발해야만 두 가지 시장에서 모두 우리나라가 제약 강국으로 도약할 수 있습니다”라고 강조했다.

한의학 세계화를 위한 노력도 이어질 전망이다. 박 부회장은 “코리안 메디슨(Korean Medicine)이란 이름으로 한의학을 브랜드화 할 예정입니다. 선진국을 포함한 동유럽, 중앙아시아 등으로 진출해 전 세계 사람들이 한의학의 혜택을 받을 수 있도록 만들고 싶습니다”라고 전했다. 한의학이 세계에서 인정받고 보편화되는 것이 그 기술과 효능에 대한 객관적인 지표 중 하나가 되리라는 생각이다.

하나된 힘으로 한의학의 도약 이끌 터
지난 2013년 4월 취임 이래 임기 3년차를 맞이하고 있는 박완수 수석부회장은 한의학의 위상을 제고하는 한편, 국민들 앞에 당당한 한의계, 국민을 위한 한의계로 거듭나기 위해 협회의 역량을 집중시키고 있다. 박 부회장은 먼저 첫 번째 핵심 사업으로서 한의사의 진단치료기기와 의료기기 사용을 꼽았다. 이는 현재 개발되어 있는 의료기기들 가운데는 한의학적 치료에 있어서 객관적인 질병진단과 예후파악에 도움을 줄 수 있는 기기들이 많다는 판단에 근거한다. 그는 “엑스레이, 초음파, 진단용 영상촬영기기 등의 의료기기 사용을 통해 한의학이 과학화, 객관화된다면 결과적으로 국민의료증진에 기여할 수 있는 계기가 되리라 생각합니다. 여론조사에서도 국민 87% 이상이 이를 지지하고 있다고 집계되었으며, 헌법재판소에서도 위법이 아니라는 판결이 나왔습니다. 국민들의 건강을 위한 일이라는 대승적 차원에서의 정부의 판단이 필요한 때입니다”라고 말했다.

두 번째로 추진하는 것은 한방 의료보험의 보장성 확대이다. 박 부회장은 “지금의 취약한 한방 의료보험 체계는 한의원을 방문하는 국민들의 부담을 전혀 덜어주지 못하고 있습니다. 저희는 보험 제도를 강화함으로써 보다 많은 분들이 한의원을 찾아 마음 편히 치료를 받을 수 있도록 돕는 일에 최선의 노력을 다하겠습니다”라고 밝혔다. 

과거 11년 간 군의관으로 복무하며 조직문화와 행정을 경험한 바 있는 박 부회장은 당시의 경험을 통해 얻은 공공업무의 노하우와 리더십을 바탕으로 ‘하나로 단합되는 협회’를 만들어나가는 데에 앞장서겠다고 다짐했다. 또한 그는 가천대학교 한의학과 교수로서 실력과 인성을 두루 갖춘 후학 양성에도 매진하고 있다. 국민들에게 더 발전된 의료 혜택을 제공하기 위해 쉼 없이 열정적인 매일을 보내고 있는 박완수 수석부회장. 그의 노력이 세계로 뻗어나갈 한의학의 도약에 든든한 발판이 되기를 바란다.

Natural substance new drugs policy needs to be revised
The Association of Korean Medicine (AKM) has been leading the fight to revise the 'natural substance new drugs policy' which has wasted more than 1.4 trillion won (approx. $1.2B). AKM expressed grave concern that the policy is deviated from original intension and it must be fully revised. First Vice Chairman Wansu Park of AKM said "The policy was to use Korean herbs to make new drugs like aspirin or taxol to pioneer global markets. When there was no substantial results after hundreds of billions of won of investment had been disappeared, the Ministry of Food and Drug Safety kept changing the contents of the policy and is trying to introduce ridiculous natural substance new drugs that might even harm public health in the end." 

AKM suggested two things: laying out the groundwork for Korean medicine and promoting the Korean medicine industry. Park said "The natural substance new drugs industry has become a highly promising industry both home and abroad and related research is being actively carried out. But many people seem to ignore the fact that the traditional medicine market is also rapidly growing. China, for example, earns about 4 trillion won (approx. $3.7B) a year by exporting traditional Chinese medicine but Korea is playing the game in a well." 

The Chinese government is actively supporting R&D and pioneering global market in pursuit of developing traditional Chinese medicine. Meanwhile, the Korean medicine industry recorded 365.4 billion won profit (approx. $320M) in 2002 and had only been going downside: 229.2 billion won in 2005 and 162.8 billion won in 2009. In addition, the National Health Insurance Service's budget for western medicine is about 10 trillion won a year while it is mere 30 billion won for Korean medicine. Park said "We should not be confused with developing new drugs and Korean medicine. Each field has its own value and the development should carried out accordingly", adding that "I want to promote the name 'Korean Medicine' as the brand name itself to pioneer the global market because we have to prove excellence of our traditional medicine in the global stage to fully recognized by the people of the world."
Leading the Korean medicine industry
Inaugurated in April 2013, First Vice Chairman Wansu Park of AKM said that 'using Korean medical doctor's diagnosis and treatment equipments' is the first core project of the association. Park said "If Korean medicine demonstrates its effectiveness scientifically and objectively with the help of the existing medical equipments such as X-Ray, ultrasound wave and video imaging, it will as much contribute to public health as western medicine. More than 87% of people support the idea in a poll and the Constitutional Court of Korea also approved its legitimacy. I hope the government will bring up with a right and mutually beneficial policy."

The second core project of AKM is expanding Korean medicine health insurance. Park said "The current Korean medicine health insurance is not helpful to reduce the financial burden of the public at all. Therefore, we want to push forward a more comprehensive health insurance policy so that more people can use Korean medicine without burdens." Meanwhile, Park. who served as army doctor for 11 years before, is now also serving as a Korean medicine professor at Gachon University.
