전방경추금속판고정술의 판도 바꾼 한국의술의 우수함
“척추외과 각 분야 협진과 융합을 이룬 세계적 척추센터건립이 목표”

세계적인 척추 권위자 중 한 명으로 꼽히는 박종범 교수는 지금까지 6,000례의 난이도 높은 경추와 요추 질환에 대한 척추수술을 집도하고 학술지 발표활동을 통해 진보적인 수술용례의 표본이 되고 있다. 세계정형외과학회의 공식학술지와 유럽척추학회의 공식학술지에서 편집위원으로 활약하고 아시아와 중동의 척추학회를 이끄는 주축으로서, 그는 정도를 지키는 치료와 수술 철학을 강조한다. 고령화 사회를 앞두고 융합형 척추센터를 만들어 국민건강에 기여할 목표로 집도와 연구에 정진하는 박 교수의 근황을 정리해본다.

당뇨와 척추건강의 상관관계 연구, 디스크수술법 개선에 성공
가톨릭대학교 의정부성모병원 정형외과 박종범 교수는 북미 척추학회에서도 관심을 갖고 있는 노령 인구의 척추이상에 대해 20년 가까이 오랫동안 연구해오고 있다. 식습관과 생활습관이 누적되어 피로도가 쌓여 척추에 문제를 일으키는데, 그 중 비중이 높은 척추디스크는 청년층에, 척추관 협착증은 고령층의 척추관절이 퇴행하면서 발생한다. 신경이 연결된 허리와 목은 무릎, 발목관절보다 건강과 일상생활에 더 큰 지장을 준다.

더욱이 혈액순환을 가로막는 당뇨는 디스크와 척추관절 및 신경에까지 나쁜 영향을 주어 척추질환을 일으키는 가장 주된 원인으로 지목된다. 당뇨가 척추질환에 미치는 영향을 논문과 수술 집도 용례로 파악한 박 교수는 연구 성과를 발표하여 많은 학회에서 수상의 영광을 안았다. 이전에도 박 교수는 목디스크의 수술방법 개선으로 주목을 받았다. 당시에는 신경을 죽이는 디스크와 뼈 조각을 제거하고 금속판으로 유합술을 하는 전방경추금속판고정술이 세계적 표준수술법이었다. 그런데 수술에 쓰이는 금속판은 수술부위 위아래에 모두 영향을 주고, 결국 척추마디들이 하나로 굳어져서 관절기능에 이상이 생긴다. 박 교수는 미국 유학 시절, 의사들조차 인식하지 못했던 문제점을 찾아내 금속판을 짧게 박아야 한다는 원리를 알아냈다.

이 이론은 현재까지 새로운 경추금속판 개발에 매진하고 있는 미국과 유럽의 대형 척추기기 회사들과 척추의사들에 의해 참고문헌으로 자주 인용하고 있다. 또한 미국정형외과학회지와 북미 척추학회지에서도 기존 방식보다 더 나은 방식으로 인정되어 주요 논문으로 게재되었다. 이러한 업적을 바탕으로 박 교수는 미국 본토 의사들도 이뤄내지 못한 미국 경추학회에서 최우수 논문상을 3번 수상하였고 유럽척추학회에서도 최우수 논문상을 아시아인으로 처음 수상했다. 현재도 허리와 목의 척추관 협착증세로 병원을 찾는 만 80세 이상의 고령 환자들의 난이도 높은 수술을 성공적으로 집도하고 있는데, 95세 환자의 퇴행성 척추관협착증에 대한 4분절 광범위 감압술 및 유합 수술을 성공시켜 권위를 입증했으며, 국내 뿐 아니라 해외에서도 인정받는 성과를 얻고 있다.

의정부성모병원의 초대 연구부원장을 2년 간 역임한 박 교수는 지난 2003년부터 경추 및 요추부 디스크, 퇴행성 척추질환 연구 성과를 인정받아 4년 간 마르퀴즈 세계인명사전에 등재와 함께 2014-2015 이년연속 세계100대의학자에 선정되고, 대한정형외과학회, 대한척추외과학회, 세계정형외과학회 및 유럽척추학회의 학술지 편집위원위원으로 왕성한 학술 활동을 하고 있다. 그 외에도 척추분야의 최고 권위를 자랑하는 스파인(SPINE)지를 발간하는 국제요추학회에서 3년 임기의 아시아 대표로 선출되어 집행위원회에 참여하고 있다. 경추 수술의 패러다임을 바꾼 박 교수는 최근 각광받는 척추관 협착증 수술에 최신 수술기법인 역동적 안정화 기법을 도입하여 수술을 최소화하면서 척추관절의 운동성을 보존하면서 빠른 증상의 회복을 얻고 있다. 이는 약 4cm정도의 작은 절개로 신경압박을 풀어주고 수술 부위의 운동성을 보전하는 수술로서 짧은 재활 기간을 거쳐 일상생활에 더 빠르게 복귀할 수 있다. 연간 400-500회의 수술을 집도하는 박 교수의 수술경과는 상당히 우수하여  서울과 타 지방의 환자들이 많이 찾고 메르스 때에도 오히려 예약이 폭주했다고 한다.

여름이나 겨울에 휴가를 낸 기억이 없다는 박 교수는 의사의 사명감에 대해 강조한다. 해외에 학술상을 받으러 갈 때에도 3박 4일의 일정을 넘겨본 적이 없다. “대학 교수직이라는 자리는 수술 뿐 아니라 진료 교육 연구논문, 기초연구에도 많은 노력이 필요하다. 초대연구부원장 시절 척추 디스크질환에서 디스크 세포가 전이성 암세포 등을 사멸시키는 연구를 하여 상을 많이 받았다. 특히 고령의 환자에서 높은 빈도로 발생하여 삶의 질을 많이 떨어뜨리는 퇴행성 척추관 협착증은 제대로 된 질병의 원인에 대한 이해와 수술이 치료 성공의 관건이다. 이 질환의 주된 원인인 황색인대의 비후가 발생하는 기전을 생화학적으로 연구한 논문들 역시 미국의 권위 있는 척추 전문지에 연달아 게재되었다고 한다.

기초와 임상 지식의 융합형 협진이 이뤄지는 전문 척추센터 건립이 목표
앞만 보고 달려온 성과를 점검하며 박 교수는 의료서비스 개선에도 많은 관심을 기울이고 있다. “외래를 비울 때 토요일에도 수술 일정을 잡는 의사로 바쁘게 살고 있지만, 환자들에게 신뢰를 받는 인간적인 의사를 모토로 삼고 있다. 신앙을 가지면서부터 그런 확신을 갖게 됐다. 의사로서 가장 중요한 것은 올바른 진단으로 환자의 증세를 잘 치료해 주고, 정확한 집도로 수술의 효과를 높이는 것이라 믿는다. 의사가 과잉진료를 줄이고 의료서비스에 대한 자정활동을 해 나가는 것도 수술의 데미지를 줄이는 역할을 한다고 본다” 국내의 장한 박사, 미국의 대니얼 류 박사 등 이 분야 권위자들과 함께 성장했으며, 바람대로 인간적인 척추 전문가로 꼽히는 박 교수는 아시아에서 척추분야의 전문 학술지가 없었을 때 대한정형외과학회 회장이신 정재윤 교수님, 삼성서울병원 이종서 교수님과 함께 아시안 스파인 저널을 창간하여 9년간 열정적으로 편집과 발행을 해 온 노력이 결실을 맺으면서 아시아와 중동 대륙의 척추학회를 대표하는 공식 학술지로 만들었다고 한다. 이 값진 성과로 바탕으로 전 세계 3대 스파인 저널이 될 때까지 키울 생각이다. 그리고 연구를 계속해 후학들을 양성하는 자양분을 만들고, 아시안 스파인 저널과 아울러 우리나라와 세계 모두에 인정받는 척추센터를 만드는 것이 박 교수의 바람이다.

박 교수는 척추수술의 90% 이상을 정형외과가 전담하는 중국과 일본보다 신경외과와 잘 연계된 한국의 조건을 십분 살려, 정형외과, 신경외과, 재활의학과 등 척추와 신경에 연계된 전문 학과간의 협조와 협진이 이루어져야 한다고 강조한다. 융합기술이 과학의 발전을 가져왔듯 척추분야의 기초와 임상 지식의 융합이 가져올 장래는 밝다고 말하는 박종범 교수. 앞으로도 가장 합리적인 척추치료 전문 국가의 명성을 얻게 되는데 기여할 박 교수의 존재는 한국 척추의료기술의 소중하고 값진 자산이다.

From research on correlation between diabetes and spine health to innovation of disc surgery
Orthopaedics Prof. Park at the Catholic University of Korea Uijeongbu St. Marry's Hospital has researched on spinal abnormality of elderly people for almost 20 years. There are various factors that can cause spinal problems including bad diet, life style and chronic fatigue. If spinal discs usually happen to young people, spinal stenosis is for older people due to the regression of spinal joints. Discs in waist and neck, however, can cause bigger problems in daily life and diabetes is known to be the main cause of spinal diseases as it affects discs, joints and the nerves badly. Park has written many research papers regarding these issues and won numerous prizes. About neck discs among the papers attracted much attention. The frontal cervical vertebral metal plate fixing, that removes discs and bone chips, and conglutinate the part with metal plate, was the common and globally standard method of surgery. The problem was that the metal plate affected the operated parts and caused disfunction of the joints. Mulling over to find a solution, while he was studying in the U.S, Park came to know the fact that inserting the metal plate short can solve the problem. This instantly created a sensation in the academic world and Park's finding has been often used as a reference by  major spine apparatus companies in the U.S. and Europe from then on. The paper was also published on the journals of American Association of Orthopaedic Surgeons and North American Spine Society as a better treating method. Thanks to his findings, Park made a record by winning three times the Best Research Paper Awards from the American Society for Cervical Vertebral and the Best Research Paper Awards from the European Spine Society for the first time as an Asian. Park also proved his authority in this field by successfully performing 'four segment wide decompression and agglutination surgery' for a 95 year old patient and he is ardently carrying out surgeries for people aged over 80.

Park served as the first Deputy Research Director of Uijeongbu St. Mary's Hospital for 2 years. He was listed in Marquis Who's Who in 2014 and 2015 (last 4 years) in recognition of his contribution to the field and was chosen as one of the world's 100 medical scientists two year in a row. Currently, he is serving as an editor of the Korean Orthopaedic Association, Korean Society of Spine Surgery, International Orthopaedic Association and European Spine Society. He is also serving as a sole Asian member of executive committee (3 year term) of International Society of Lumbar Vertebrae which runs the world's top spine journal SPINE. Park's achievements did not stop here. As the top authority who changed the paradigm of spinal surgery, Park applied topnotch 'dynamic stability method' to spinal canal stenosis surgery. With a little 4cm incision, it removes the pressure on the nerves, minimizes surgery procedure, saves the motility of operated parts and enhances fast recovery. Due to a high number of surgery (400-500 times a year), Park rarely have had a holiday which lasts no more than 3 days even if it is an academic trip abroad. Park said "Being a professor means a lot of work. Thanks to my diligence, I won numerous prizes with one for finding the fact that the spine disc cell can kill cancer cell. Especially, degenerative spinal canal stenosis, normally occurring to elderly people, requires a right understanding about the cause and a high level of surgery. The mechanism that occurs hyperplasia of swollen yellow ligament is one of the main causes of the disease and my research papers on the subject also was published on numerous top US academic journals.

Meanwhile, Park is paying much attention to improvement of medical services. Park said "As a doctor, I want build trust with my patients by providing correct diagnosis, treatment and surgery and removing all the unnecessary options in pursuit of making more profits." Park grew up with some of the authorities in the field including Dr. Han Jang (Korean) and Daniel Ryu(U.S.). He founded the top Asian spine academic journal 'Asian Spine Journal' in cooperation with prof. Jaewoon Jeong (Chairman of Korea Orthopaedic Association) and prof. Jeongseo Lee of Samsung Medical Center and he has served as editor for 9 years. Park wants to make the journal as one of the top three spine journals in the world. However, his bigger ambition is building the world's best spine center in Korea. According to Park, more than 90% of spinal surgery is conducted by orthopaedics in the case of China and Japan but it is closely related to neurosurgery in Korea. Park's idea is to build a comprehensive surgery method by combining orthopaedics, neurosurgery and rehabilitative medicine. Like convergence of modern technology has brought us innovation, Park added, convergence of knowledge on spine and clinical test will bring us a brighter future in the field.
