양·한방 협진 전문인력이 상주하는 ‘즐거운 병원’
“초고령사회를 대비해 요양원과 병원의 장점을 고루 갖추다”

현 정부의 국정비전인 ‘국민행복시대’는 은퇴한 노령인구에도 적용된다. 질병에 취약하고 각별한 보호가 필요한 노인들은 가정과 사회에서 편안하게 머물 공간을 찾기가 쉽지 않다. 따라서 요양원과 실버타운, 도심지의 전문병원과 구분되는 요양병원은 병원처럼 의료진의 관찰과 검진을 받으며 인생의 황혼을 맞이하는 공간으로서 주목받고 있다. 전문성 있고 고객 편의를 존중하는 다정다감한 요양병원을 추구하는 박한규 병원장을 만나 보았다.

고령 환자 가족이 믿고 의지할 수 있는 전문요양병원
평균수명의 증가는 경제활동 연령대가 지난 노인들과 가족들에게는 부담이 된다. 2030년에는 인구의 30%가 노인이라는 초고령사회가 예상된다. 과거 사회와 달리 핵가족화된 현대 사회에서는 질병이 있거나 65세 이상의 20%가 치매를 앓고 있는 고령인구들이 혼자 거주한다는 것도 가족에게 근심이 된다. 건강한 노인들은 서로 교류하는 실버타운에 가고, 큰 질환이 없는 노인들은 요양원에서 건강 식단으로 짜여진 평화로운 일상을 보내게 된다. 보다 면밀한 관찰과 치료가 필요한 고령인구들은 요양병원을 찾는다. 

2011년 3월 설립되어 보건복지부 인증제를 통과하고 총 220여 병상을 갖춘 늘푸른요양병원은 수도권과 거리가 있는 고즈넉한 지역, 인구수 18만 명의 진해구에 위치한 요양병원 중에서도 높은 지지와 신뢰를 얻고 있다. 이곳의 박한규 병원장은 양방과 한방 협진으로 밤늦은 시간, 그리고 주말 근무도 마다하지 않고 환자들을 면밀히 살피는 의료인이자 대한흉부심장혈관외과학회 소속 정회원이기도 하다. “이곳은 벚꽃축제로 유명한데다 산과 바다가 가까우며 풍경 좋은 곳에 위치해 있어 환자 가족들이 안심하고 어르신을 맡기신다. 우리 병원은 진해구의 요양병원 중에서도 설비나 운영 충실성에서 근무 의료진과 직원, 환자들과 그 가족들이 모두 인정한다고 자부한다. 대부분의 요양병원에서는 치료와 휴식 커리큘럼에 충실하다면, 우리는 사회복지사의 정기적인 방문과 어르신들의 취향에 맞춘 행사를 꾸준히 갖고 있다. 또한 직원에 대한 복지에도 충실하니 기분 좋은 일터가 되고, 직원들의 행복이 환자들에게도 전해져 긍정적인 영향을 준다고 생각한다.” 

요양병원에 대한 의식제고와 선택의 기준
늘푸른요양병원은 실버타운과 요양원, 그리고 치료가 접목된 병원을 합한 형태로 보건복지부 의료기관 인증 외에도 지난해 제52회 소방의날 「소방안전부문」 안전행정부장관상을 수상하며 의료서비스와 시설 면에서도 우수하다는 평가를 받고 있다. 
고령화시대를 맞이하여 요양병원이 우후죽순 생겨나고 있지만, 초기의 느슨한 허가로 인해 비정상적인 운영체계가 잡힌 곳도 있다. 그러한 점을 뒤늦게 규제하려고 하니 정석대로 운영하는 요양병원들이 타격을 입기도 한다. 늘푸른요양병원은 이러한 애로사항을 전문성과 차별화된 경영방침으로 극복해가고 있다.

박 병원장은 “입원을 위해 원무과에 상담하러 오시는 분들이 종종 혼동하시는 부분이 있다. 요양원과 요양병원의 개념은 다르다. 요양원에서는 상담과 간병을 겸하는 간병사가 있는데, 요양병원에는 반드시 자격을 갖춘 의료진, 간호사가 상주한다. 요양원은 의료진이 없으니 환자당 간병사 몇 명을 보유해야 한다는 필수기준이 있다”고 언급한다. 그러나 요양인원으로서 필수적인 간병사는 보건복지부에서 의료법상 인정이 안 되는 부분이므로, 의료수가 혜택 등에서 사각지대에 있다. 경제적으로는 비용이 발생하지만, 보험이 있는 의료비와 달리 100% 환자와 보호자가 부담해야 한다는 뜻이다. “좋은 요양병원을 선택하는 기준이 궁금하실 것이다. 가급적이면 보호자 입장에서 자주 찾아뵐 수 있는 지역을 직접 알아보는 게 좋다. 그리고 적정 비용을 정할 때, 지나치게 낮은 비용이라면 관리가 잘 되지 않는 결과로 이어진다. 또 하나는 대개 입소문이나 검색으로 찾아오시는 분들이다 보니 병원 분위기, 공간이 환자와 맞는지도 둘러보시고 결정해야 한다는 점이다. 너무 밀집된 형태의 병상도 피하시는 것이 좋다. 그리고 의료진이 없는 요양원처럼 만성당뇨 환자가 장기적으로 방치되는 예는 없지만 행위별 수가가 적용되어 철저한 회진으로 관리되는 병원과는 차이가 있다. 그러니 주변에 다른 큰 병원과 연결되어 운영되는지도 잘 따져보셨으면 한다.” 

가족을 모시고자 하는 마음으로···
박 병원장은 이러한 기준을 경영에도 적용하고 있다. 또한 매달 첫째 주는 어르신들을 위한 생신잔치, 셋째 주 수요일은 타 지역에서 온 가수들의 공연 행사를 진행해 환자 보호자들로부터 “다른 요양병원 대신 이곳을 선택하기를 잘했다. 다른 곳에 있을 때보다 부모님의 표정이 더 밝아지셨다”는 감사 인사를 종종 듣는다고 한다. 그는 이러한 덕담을 요양병원 직원들의 공으로 돌린다. “우리 행정직원, 간병사들과 간호사들은 자발적으로 환자들에 대한 서비스를 연구한다. 간호사들은 겨울에 생강을 사다가 직접 차를 담가 나눠드리기도 하고, 악보를 만들어 와 어르신들이 좋아하던 노래를 함께 불러 드린다. 또 거동이 나아지신 분들을 대상으로 글을 가르쳐드리거나 그림그리기, 일기쓰기를 하시도록 한다. 한 병동당 한 분씩 선정하여 생신잔치 때 상으로 선물을 드리는데 반응이 좋다.”
봄과 가을에는 휠체어나 차로 이동 가능한 어르신들을 산이나 바다로 모셔가 따뜻한 오후 시간동안 경치를 볼 수 있게 배려한다고 한다. 이에 박 병원장은 “의료적 치료뿐 아니라 정신건강을 지켜 드리는 것도 중요하기에, 전문성과 더불어 내실과 재미를 갖춘 병원을 추구하고 있다”고 말했다. 또한 “앞으로도 어르신 환자들이 원하는 기쁨을 드리고자 한다. 어르신들 사이에는 미리 영정사진을 찍어두면 장수한다는 속설이 있다. 그래서 영정사진을 액자로 만들어드리는 것을 고려하고 있다”고 말했다. 

마지막으로 박 병원장은 최근 문제가 된 보육교사의 자질 문제처럼 요양병원에 대한 무분별한 편견과 규제를 우려하면서, 처음부터 관련업계 종사자에 대한 정상적인 양성과 의료수가에 대한 배려, 그리고 적합한 대책이 있기를 바란다는 뜻을 밝혔다.

Geriatric where families of elderly patients can rely on
The increased average life span can be a burden to retirees and their family. It is expected that 30% of the population will be consisted of elderly people by 2030. Also notable is that many elderly people are suffering from various diseases and more than 20% of above 65 age group is suffering from dementia and living alone which is a worry for their family members. If they are healthy, they can spend time in a sliver town, and if they are not, they usually go to a sanatorium. In the case of those who suffer from a severe pain go to geriatric hospital. Evergreen Geriatric Hospital is founded in March 2011 and passed an approval of Ministry of Health and Welfare and has 220 wards. It is located in Jinhae Distric (180,000 population), Changwon City and it is a highly praised geriatric hospital in the region. The director Park is a member of Korea Chest Cardiovascular Surgery Society and he is working really hard for patients in cooperation with western and Korean medical specialists. Park said "Jinhae is famous for cherry blossom festival. There is mountains behind and the sea in front and families of elderly patents are happy with the location. I can proudly say that our hospital has excellent facilities, management and staff, and this was also proved by many of our customers. Most geriatric hospitals are good at providing a treatment and rest, but we regularly hold various events such as social worker's regular visit to elderly people in addition to that. We also has a good welfare system for our staff and this is being resulted in providing excellent customer services."

The standard of choosing a right geriatric hospital and its recognition
Evergreen Geriatric Hospital combined the functions of silver town, sanatorium and treatment and their excellence was proved by winning the Minister of Government Administration and Home Affairs's Prize in 'Safety of Fire Fighting Sector' at The 52nd Fire Fighter's Day last year for excellent medical services and facilities. Many geriatric hospitals are running their business at the moment but not all of them have a proper management system due to the lack of regulation at the first place. And the improvement of the regulation sometimes can cause a damage to well standardized hospitals. Evergreen Geriatric Hospital is overcoming these problems with differentiated management policy and professionalism. Park said "there is a part that patents are often confused. The conception of sanatorium and geriatric hospital is different. The first one has personal support workers but the latter one has certified nurses and specialists." But personal support workers are not approved in terms of medical law which means that the patents and the families have to pay 100% of medical fee. Park added "the standard of choosing a good geriatric hospital is the location where the families can easily access. And if medical fee is too low, it means that the hospital is not properly managed. Another factor is that the patients and families have to visit and have a look around the hospital before the final decision. It's good to avoid a packed ward. Also you need to check whether it is managed thoroughly and has a good connection with general hospitals." 

Treating patients like my family
Park holds a birth day party for elderly people in the first week of every month and invite singers to an event for patients on Wednesday in the third week. For this effort, the family members of patients are impressed. But Park attribute this compliment to his staff. Park said "our staff are spontaneously researching on customer services. Nurses make ginger tea for their patients in winter, make a sheet music and sing the songs they like and teach writing a diary and painting. We also give a birthday present and are receiving good feedback." They take patients who have difficulty in moving to a mountain or the sea on a wheelchair or on a car. Park said "mental health is as important as physical health. We try to build a hospital which has professionalism and internal stability. There is a saying that preparing a portrait photo for funeral earlier can lead to a longevity and we are considering to make it into a frame." Meanwhile, Park worried about prejudice and regulation on geriatric hospitals and hope there will be appropriate methods for fostering specialists and policies.
