약효 지속시간 크게 향상시킨 통풍치료제 개발
인체에 무해한 알부민 결합시켜 단백질 의약품의 혁신 제시

의료기술의 발전으로 인류의 기대수명은 늘어났지만, 오히려 현대인의 건강은 갈수록 악화되고 있다. 특히 운동 부족 같은 불규칙한 생활습관과 식생활의 서구화는 세대를 뛰어넘어 각종 성인병과 노인병이 발병하는 원인을 제공하고 있다. 통풍은 중장년층뿐만 아니라 최근 20~30대의 젊은층에게서도 많이 나타는 질병이 되며 큰 문제가 되고 있다.

요산분해효소의 지속성 늘린 신기술
몸 안에 요산이라는 물질이 과도하게 축적되어 발생하는 통풍은 관절의 연골, 힘줄에서 결정화되어 붓고 염증을 일으키며, 심하면 콩팥장애, 결석, 심장질환, 당뇨, 고혈압과 같은 합병증으로 이어지는 원인이 된다. 혈액 속에 요산의 양이 일정 수치 이상으로 높아지는 이유는 과도하게 섭취된 ‘퓨린’이라는 물질이 분해되어 요산이 생기기 때문이다. 이 같은 현상을 치료하기 위해 지난 2010년 미국 식약청에서는 ‘요산분해효소(urate oxidase)’를 통풍치료제로 허가했다. 요산분해효소는 요산을 인체에 무해한 물질로 바꾸는 효과를 가지고 있다.

하지만 자연계에서 얻어지는 요산분해효소는 체내에서 빨리 분해되어버려 약효를 오래 유지하기 어려운 문제점을 가지고 있었다. 이를 해결하기 위해 요산분해효소에 합성고분자인 폴리에틸렌글라이콜을 결합시킨 의약품이 개발되었으나, 이 성분에 대한 면역 반응이 발견되었으며, 약효 또한 감소된다는 사실이 보고되면서 대체 기술의 개발이 시급히 요구되어 왔다. 

광주과학기술원 ‘생분자 공학 및 바이오 융합 재료 실험실’의 권인찬 교수는 최근 요산분해효소의 약효가 체내에서 오래 지속되도록 하는 새로운 기술을 개발하여 향후 통풍을 치료하는데 중요한 방법을 제시해 학계의 주목을 받고 있다. 권 교수는 “단백질 의약은 합성 의약품에 비해 약효도 좋고, 부작용이 적다는 장점을 가집니다. 하지만 체내에서 빨리 제거되기 때문에 여러 번에 걸쳐 투여를 반복해야 되며 치료비용을 크게 증가시키는 원인이 되어 왔습니다. 저희는 기존에 연구되었던 합성 고분자를 결합하는 대신 사람에게서 얻어지는 안전한 단백질인 알부민을 요산분해효소에 결합시키는 방법을 연구했습니다”라고 설명했다. 이 연구는 약물전달 분야의 권위지인 저널 어브 콘트롤드 릴리즈(Journal of Controlled Release) 4월 15일자 온라인판에 게재되었다.

향후 단백질 의약품 기능 향상에 크게 기여
생체세포나 체액 중에 넓게 분포되어 있는 단백질인 알부민은 사람의 혈액 속에 이미 풍부하게 존재하고 있기 때문에 해가 없을 뿐 아니라, 체내에서 오랜 시간 안정적으로 존재하는 성질을 가지고 있다. 하지만 기존의 기술을 사용하여 알부민을 요산분해효소와 무작위로 결합을 시키면, 약효를 내는 중요한 분위가 가려져서 약효가 크게 감소한다는 문제가 발생했다. 

연구팀은 이를 해결하기 위해 요산분해효소의 약효에 영향을 주지 않는 특정 위치에만 알부민을 결합시킬 수 있는 기술을 최초로 개발했다. 권 교수는 “자연계에 존재하지 않는 반응기를 가지고 있는 아미노산을 요산분해효소의 특정 위치에 도입한 후에 알부민을 결합시키는 기술이 사용되었습니다. 이를 통해 알부민이 결합되어도 요산분해효소의 약효가 감소하지 않는 것을 확인했으며, 동물실험을 통해 요산분해효소의 약효가 체내에서 7배나 오래 지속된다는 것을 확인했습니다”라고 밝혔다.

실험실에서는 그동안 단백질 의약을 이용해 질병을 치료함으로써 인간의 건강을 증진시킬 수 있고, 생촉매인 효소를 이용한 친환경적인 전환공정을 실현하기 위한 전략 개발 등을 주로 연구해왔다. 이를 실현시키기 위해 단백질 의약과 생촉매를 이용해 기능적으로 향상된 바이오 융합 재료 합성방법을 연구해왔으며, 이러한 연구의 결과로 알부민-요산분해효소 결합물을 개발해낼 수 있었다. 

권 교수는 “이번 연구 성과는 통풍 치료뿐만 아니라 향후 다양한 단백질 의약품의 기능을 향상시키는 데에 적용할 수 있을 것으로 기대되고 있습니다. 실용화를 위해서는 임상실험 등의 과정이 필요하지만, 기존 의약품보다 약효가 향상된 신약이기 때문에 실용화의 가능성은 더없이 높은 것으로 평가됩니다”라고 전망했다. 이들이 개발해 낸 의약품이 더 많은 이들의 건강 증진과 삶의 질 향상에 크게 기여할 수 있게 되기를 기대한다.

New technology that increased durability of uric acid catabolic enzyme
Gout is the result of the excessive accumulation of uric acid in the body. It crystalized onto tendon to cause inflammation, renal disorder, stone, cardiac disorder, diabetes and high blood pressure. The reason the amount of uric acid increases more than the standard is oversupplied purine that creates uric acid through decomposing. To solve this problem, U.S. FDA allowed Urate Oxidase to be used for curing gout. The uric acid catabolic enzyme changes uric acid to unharmful substance. But the enzyme gained from nature gets decomposed quickly which is a problem to maintain the medicinal effect longer. A medicine combined with the enzyme and polyethylene glycol was developed to solve the problem but the tolerance and weak medicinal effect have been reported.  

But the hope came from Biomolecule Engineering & Bio Convergence Materials Research Institute. The leader of the institute Gwon developed the medicine that lasts longer in the body and is attracting many people from academic circles. Gwon says "protein medicines are more effective than synthetic medicines with less side effect. But it was problem that it gets removed quickly and you need to keep taking it in which increases the treatment cost. So we combined albumin gained from human body with the enzyme instead of using synthetic high molecule to solve the problem." Gwon's paper was published on Journal of Controlled Release on 15 April.

Contribution to improving the function of protein medicines
Albumin is a kind of protein widely spread in cells and body fluid, abundant in the blood and stays there for a long time. But if it is carelessly combined with the enzyme through the existing technologies, it can reduce its medicinal effectiveness. Gwon says "amino acid has an effector which is non existent in nature. What we did was that we used the technology which brings amino acid in a certain position to combine it with albumin. We confirmed that the medicinal effect was not at all reduced when albumin is combined with amino acid and it stays in the body for 7 times longer than the existing medicines after animal tests."

The institute has been exerting its effort to treat diseases by using protein medicine and to realize eco-friendly medicinal conversion process. They researched on combining methods by using protein medicine and bio catalyst and they could be able to develop albumin-uric acid catabolic enzyme as a result. 

Gwon added "our achievement is expected not only to be used for curing gout but also to be applied to improve various protein medicines. We still need clinical test in order to commercialize it but we are positive that it won't have any problems to be on the market."
