마음의 상처까지 보듬어주는 오직 화상 환자들을 위한 화상전문병원

최근 담양군의 한 펜션에서 화재가 나 같은 대학 동아리 학생들이 크게 다치는 사고가 일어났다. 그 중 한명은 이틀 뒤 결혼식을 올릴 예정이었다는 사연이 뒤늦게 보도되면서 보는 이들의 마음을 더욱 아프게 했다. 부쩍 건조해진 날씨 탓에 더욱 빈번하게 일어나는 화재 사고로 화상 환자들의 숫자가 크게 늘어나고 있는데 전문병원이 턱없이 부족하다는 지적이 나오고 있다. 특히 화상전문병원이 전무한 지역에서는 화상 환자들이 서울을 비롯한 대도시로 이동하여 치료를 받아야 하고, 그 과정에서 적절한 치료시기를 놓쳐 경과가 좋지 않거나 큰 흉터를 남기게 되는 사례가 많아 더 큰 문제로 대두되고 있다. 대한민국 화상치료의 새로운 장을 열고 있는 열린문외과의원의 문성민 원장을 만나 자세한 이야기를 나눠봤다. 

화상치료에 취약한 안산·시화 지역에 체계화된 치료의 길 열어  

지난 9월 경기도 안산시 상록구에 개원한 열린문외과의원은 지역 최초의 화상전문병원이다. 이곳의 문성민 원장은 항상 이어마이크를 착용하고 병원을 안을 누비는데, 외래환자부터 입원치료 중인 환자들의 상태를 제때 파악하고 그들의 불편함을 신속히 해결하기에 하루는 너무도 짧기 때문이라고 했다. 최소 3~4년 이상 화상치료를 도와온 다른 스텝들의 움직임도 문 원장의 템포에 맞추어 환자들의 치료를 위해 최상의 하모니를 만들고 있다. 의사들도 어려워한다는 화상치료지만 아픔에 눈물짖는 환자들이 있는 한 누군가는 꼭 해내야 하는 일이기에 고된 줄도 모르고 달리는 여정이다. “지역에 체계화된 화상치료와 외과 환자들을 위한 병원이 필요하다고 보고 개원을 하게 되었죠. 무엇보다 치료를 위해 환자들이 서울 혹은 타 지역으로 이동이 불가피했던 점을 개선하여 촌각을 다투는 화상치료가 조기에 이루어질 수 있도록 노력하고 있습니다.” 

문성민 원장은 경북 영주 성누가병원 외과과장, 경북고령영생병원 외과과장 및 진료부장, 인천 성민병원 외과과장을 거쳐 안산 한사랑병원 외과과장 및 화상전담의로 오랜 시간 화상 환자들과 함께 울고 웃었다. 현재는 경북대학교와 인하대학교에서 외래교수로도 활약 중이다. 열린문외과의원은 환자들이 보다 신속하고 정확한 치료를 받을 수 있도록 최첨단 치료 제재를 구비하고, 치료 후 피부재활 및 기타 재활치료를 위한 물리치료실을 준비하였다. 또한 입원치료가 필요한 환자들을 위해 자체적으로 입원실을 함께 운영 중에 있다. 화상치료 외에도 욕창치료, 탈장, 항문질환, 정맥류, 비뇨기 수술 등 기존과는 차별화된 시스템으로 환자들을 맞이하고 있다. “충분한 상담을 통해 화상치료와 재활, 물리치료 등을 진행하고 있는데, 화상의 원인과 손상정도, 환자의 몸과 피부상태를 고려해 최상의 진료를 추구하고 있습니다. 화상치료가 힘들고 화상으로 얻은 흉터가 마음의 상처로 남을 수 있는 만큼 의사로서 환자들의 마음을 먼저 보듬는 것이 가장 중요하다고 생각됩니다.”     

화상 흉터 그대로 방치하지 말고 반드시 치료해야 한다 

열린문외과의원의 화상치료 단계를 살펴보면 열기를 식혀주는 치료 제재인 번실드라로 상처를 덮어 상처 부위를 식혀준 후 붕대를 감고 하루를 지내는 것으로 시작된다. 이튿날부터는 화상이 심할 경우 물집이 잡히는데 그 물집을 깨끗이 제거하여 밑에 있는 살이 제대로 차오를 수 있도록 물집제거 및 삼출물 배액 과정을 거치고, 이후 피부가 잘 재생될 수 있도록 칼로덤으로 상처를 덮은 후 약 4~5일을 그대로 유지하게 된다. 다음은 피부재활치료 단계로 접어드는데 흉터 완화, 보습, 레이저 물리치료 등의 치료 단계를 거쳐 피부가 정상 상태로 돌아올 수 있게 한다. 마지막 단계는 화상 후유증 치료로 가려움, 과색소증, 저색소증, 켈로이드 형성 등의 후유증을 치료하는 단계로 물리치료와 약물치료를 병행하여 피부 재활에 힘쓴다. “요즘은 피부 줄기세포를 이용한 칼로덤이라는 재생 치료 제재가 있는데 피부가 상당히 빨리 올라오는 효과가 있습니다. 적절한 약물치료와 물리치료를 함께하면 오래된 화상 흉터도 말끔하게 치료가 가능합니다.” 

대다수의 사람들이 생활 속에서 가벼운 화상으로 인해 생긴 흉터를 제거해야겠다는 생각을 하지 못한다. 흉터를 적극적으로 제거하고자 노력하는 사람들도 대부분 의료적인 이유보다 미용적인 목적이 큰 경우가 더 많다. 이에 문 원장은 반드시 화상 흉터를 제거해야 한다고 강조한다. “화상으로 인한 흉터가 수년이 지나면 피부암을 발생시킬 우려가 있다는 것을 잊어서는 안됩니다. 특히 화상 흉터가 손가락이나 관절에 생기면 기능적인 제한이 오기 마련인데, 더 늦기 전에 제거수술을 받고 흉터를 최소화하여 차후 발생할 수 있는 많은 문제들로부터 벗어나시길 바랍니다.” 덧붙여 검증되지 않은 민간요법을 맹신하지 말고, 화상을 입었다면 해당 부위를 찬물에 여러 번 씻고 아프더라도 꼭 젖은 수건으로 감싼 채 전문병원에 내원해 줄 것을 강조했다. 한편 화상치료에 대한 의료수가문제와 화상전문병원 부족 사태에 대해 문 원장은 우려의 목소리를 높였다. “전문의로서 제가 할 수 있는 최선은 현장에서 환자를 돌보며 의사로서의 책임을 다하는 것뿐이죠. 각자의 역할이 있다고 생각합니다. 고통 받는 화상 환자들을 위해 화상치료에 대한 정부 차원의 관심과 현실성 있는 제도 마련, 건강보험공단의 적극적인 업무지원이 필요하다고 봅니다.”

화상치료에 매진해 오며 지내온 시간보다 많은 환자들을 돌보며 나아가야 할 시간이 많다며 걱정하는 문 원장의 읍소에 대한민국 화상치료 분야의 밝은 미래를 확신해 본다.

Recently a leisure pension in Damyang County was on fire and some of the university students who stayed there were seriously injured. And it was really sad that one of the students was expected to get married soon. Due to the dry weather 

fire accidents have often occured these days and people are worried that there are not many burns specialized hospitals. The patients have to move to a big city if there is not a specialized hospital nearby and it leaves permanent scars on their body during the transfer. Korea's top bilingual magazine Power Korea has met Seongmin Moon, the director of YEALINMOON Surgery and learned about the system.

Systematic ways to treat burns in Ansan area

YEALINMOON Surgery, opened in last September, is the first burns specialized hospital in Sangnok-district, Ansan city. The director Moon is always wearing an ear microphone when working in order to check and respond to the patient's states and needs promptly. And this is backed up by the staff who have at least more than 3 years of experience in the field. Treating burns is difficult even for the doctors but somebody must do it for the suffering patients. Moon said "I opened the surgery because I thought we need a specialized hospital in the area. Prompt treatment is crucial for burn patients but the patients had to move up to Seoul for a treatment."

Moon worked as the chief surgeon and a burn specialist in St. Luke Hospital in Yeongju city, Gyeongbuk Goryeong Youngsang Hospital in Goryeong county, Incheon Sungmin Hospital in Incheon city, and Ansan Hansarang Hospital in Ansan city. He is also active as an outside professor at Kyungbook National University and Inha University. YEALINMOON Surgery has state of the art medical equipments, physical therapy department for skin rehabilitation and a ward. They also provide treatments for bedsore, hernia, anal disease, varicose vein and urinary system problems. Moon said "we are carrying out the treatments through a thorough consultation based on the cause of burn, damage level and the patient's body and skin condition. Treating burns is very difficult and the scars give patients a metal wound so to treat their metal stability first is most important."

Burn scar must be treated

YEALINMOON Surgery's first stage to treat the burn is to cool down the affected area with burnsheild, bandage it and leave it over a day. If the burn is deep it causes blisters the next day so it has to be removed by covering the area with kalodem and leaving it for four to five days for the new flesh to sprout. The next stage is skin rehabilitation for normalization of the skin which includes the treatments like easing the scar, moisturization and laser physiotherapy. The last stage is aftereffect treatment for itch, hyperchromatism, hypochromatism and keloid formation. Moon said "there is a skin regenerating material called kalodem made from skin stem cell and it is really effective. If it is combined with an appropriate medication and physiotherapy even the old burn scars can be cured."

Most people tend to ignore treating small burn scars in daily life. Also many people want to remove the scars for a beauty purpose. But Moon emphasized it is very important to remove it by saying "the untreated burn scars can cause skin cancer. Especially when the scar is on fingers or joints there will be functional limitation so it must be removed to prevent further sufferings." He added that folk remedy is not trustworthy so people must rinse the scars with cold water several times, bandage it with a wet towel and go to the hospital as quickly as possible.

Meanwhile, Moon is worried about the charge for medical treatment and lack of burns specialized hospitals by saying "what I can do best as a specialist is to treat the patients but we need the government's interest to build a practical system for burn treatment and also National Health Insurance Corporation's business support."

Moon added that he would like to help more patients in the future and Power Korea could see a bright future in the burn treatment field through YEALINMOON Surgery.
